Hunter is a male character. He briefly had sexual reassignment surgery and became female as a part of a plot. He is back to being a male character.
Although her sexual reassignment operation was reversed, she still frequently laments her current body and desires a female one. Additionally, before the Black Hearts were even a plot point she was frequently shown to eclusively use feminine disguises. In my opinion this clearly means she is a transgender woman. She likely doesn't transition/reversed her transition merely because her career would suffer if she were to live as a woman. How many women have we seen working for the various agencies Gathers has been working for, let alone transgender ones?

We do not currently have sufficient evidence to declare Hunter a transgender woman. That would be compelling, though not wholly original as it would parallel the similar plot point on the show Twin Peaks where David Duchovny's transgender character Denise Bryson becomes Chief of Staff of the FBI. The episodes do not back up that view, though. While Hunter Gathers frequently laments missing his "beautiful breasts", he does not identify himself as female nor does anyone else following his re-transitioning to a man when he becomes head ofS.P.H.I.N.X.
The idea that staying a woman would adversely affect his career is also not credible within the Venture universe, as Gathers was the leader of the organization S.P.H.I.N.X. with nobody above him to answer to and had unwavering loyalty from his troops regardless of his gender or orientation (the troops even included characters like Shore Leave, who had been fired from the O.S.I. for being gay and should be sympathetic to work-related discrimination). Gathers only left S.P.H.I.N.X. upon being promoted to the head of O.S.I. by General Treister, who cared only about Gathers' skillset. Out of all the characters in the Venture universe, Gathers is one of the most powerful and the least likely to be hindered by transphobia in the workplace.
Finally, one of Gathers' most recent appearances was in the Season 7 episode The Inamorata Consequence, where the Second Summit of Tolerance was held in the remains of The Venture Compound. Moderater Dr. Thaddeus Venture called for decorum amidst the squabbling, referring to the collective group repeatedly as "Gentlemen!" Dr. Mrs. The Monarch complains repeatedly about being referred to as male (as she is the only cis female present), while General Gathers does nothing to acknowledge that Venture is misgendering him. Gathers does not appear to identify as female.
For the sake of both accuracy and clarity I recommend referring to Hunter with male pronouns when he has a male body and female pronouns when she has a female body. Absent more information, anything else is mere speculation about what's in the character's head.
